Surfaces or objects that are contaminated with norovirus can be cleaned with a high-level disinfectant like bleach, per Ostrosky. Though the virus’s origin remains unknown, news of the outbreak has been shared with the Centers for Disease Control and Prevention (CDC). The Silversea incident marks the fourth such outbreak in 2024, according to CDC data. Talk to a doctor or pharmacist about any medications you should pack, such as loperamide (Imodium) to help treat diarrhea or dimenhydrinate (Dramamine, Gravol) for nausea.

Outbreaks 2018 reports

It's a very common, highly contagious, ruthlessly efficient and uncomfortably bad virus affecting the stomach and large intestines. Often called "stomach flu" (the med term is "Gastroenteritis") the infection results in massive vomiting and diarrhea. Sickness outbreaks are considered as such if the percentage of infected people is over 3%. The virus is not seasonal and usually not serious (in medical terms). It hits 1 in 5 people annually and is the cause of ~50% of all foodborne illness outbreaks in the USA and for ~90% of all non-bacterial gastroenteritis worldwide. More than 90% of cruise ship gastro outbreaks are caused by norovirus, which is spread from person to person, and through contaminated objects or contaminated food or water.

How does norovirus spread?

From personal experience, food tongs are handled by multiple people, some of whom may not have cleaned their hands. The risk of getting sick with gastro was significantly higher on bigger ships and longer voyages. This is because the longer you are in close contact with others, the greater the chance of exposure to an infectious dose of viruses or bacteria. However, COVID generally presents a lesser risk nowadays, with most people having some level of immunity from vaccination or previous infection.

And while interest in cruising is down slightly among the 50-plus population this year compared to last, a recent AAA survey finds that, overall, the share of travelers considering a cruise vacation in 2023 is up. The Silver Nova, the cruise company's newest liner introduced in 2023, boasts butler service for each suite and a ratio of one crew member for every 1.3 guests. Prices for the 16-day excursion started at $11,700 for a double occupancy room, according to CruiseMapper.

Norovirus is highly infectious, as I’ve described for Forbes previously. Plus, hand sanitizer and cleaning materials that don’t contain something more powerful like bleach may not get rid of the virus from a surface. “To maintain an environment that supports the highest levels of health and safety onboard our ships, we implement rigorous cleaning procedures, many of which far exceed public health guidelines,” the spokesperson added. This is one of the most well-known bugs that can foil fun on a ship. Norovirus — marked by diarrhea, vomiting, nausea and stomach pain — is to blame for more than 90 percent of diarrheal disease outbreaks on cruises, according to the CDC. That said, norovirus outbreaks on ships account for only 1 percent of all such reported cases.

In cases of severe emergencies, passengers with life-threatening illnesses or injuries are evacuated off the ship - either by boat (if in or near seaport) or by rescue helicopter dispatched from nearest coast guard or other station. In some cases, nearby ships with better medical facilities may assist. Some cruise lines offer hand-sanitizer dispensers near the ship's restaurants, Lido/pool deck areas and other more crowded public spaces in their effort to keep a lid on sickness outbreaks. In the past 10 years, only three other cruise ships have received sanitation scores below 70, CDC records show. Violations can include something as small as not posting a raw-eggs advisory on the omelet station or a single fly hovering in a food area.

Historically, troop transport ships also helped to spread the lethal 1918 flu virus between continents. Cruises are back up to pre-pandemic levels — and so are onboard stomach bug outbreaks. The outbreak was reported one day before the “Easter” themed cruise was scheduled to end. Nearly 30 people aboard a luxury cruise liner known for its phenomenal cuisine have fallen ill thanks to a gastrointestinal outbreak that swept through the ship. Hundreds of passengers and crew members fell ill while on board a Princess Cruises ship, the Centers for Disease Control and Prevention said. It also “increased cleaning and disinfection procedures,” the agency said. Measles, a highly contagious virus that can linger in the air even hours after an infected person leaves the room, was declared eliminated from the U.S. in 2000, thanks to a successful vaccine program. But cases still pop up in the States, and the virus is common in many countries around the world. Passengers may come from multiple countries, potentially bringing variants from different parts of the world.

Most of the infections are still of unknown origin, while in three cases the outbreaks were identified as norovirus, which is a relatively common cause of gastrointestinal disease on cruises. Norovirus survives on surfaces, and it can spread easily on ships where rooms are close, buffets proliferate, and people share confined spaces. It’s really not that surprising that norovirus outbreaks have already reached a high water mark this year. As many Covid-19 precautions went the way of sweater vests in late 2022, it’s probably not a coincidence that this past winter saw a surge of norovirus cases in general, as I reported for Forbes on February 10, 2023. On top of that, cruise ships returned to full operations for the entire calendar for the first time since early 2020.

According to the cruise line, its next sailing to Grand Turk and Bimini on October 13 proceeded as scheduled. The Vessel Sanitation Program under the CDC will continue monitoring the situation, including the ship’s response to the outbreak and sanitation measures. However, antihistamines can interact with other medications and often cause drowsiness and decreased mental alertness, and the nonsedating ones appear to be less effective, the CDC says.

The cases are reported before arriving to the US from a foreign port, or whenever 3% or more of cruise passengers or crew are ill. Shipboard hospitals as facilities and equipment vary by cruise line and vessel. Clinic's size usually depends on the ship's capacity (passengers plus crew). Generally, bigger and newer ships offer larger and better-equipped medical facilities and are served by bigger infirmary staff.
